A Community Health Assessment (CHA) is a process used by health departments and community partners to collect and analyze data about the health needs, strengths, and concerns of a community. A Community Health Improvement Plan (CHIP) is the strategic action plan that follows the CHA, outlining the goals, priorities, and collaborative strategies the community will use to address the issues identified. Together, the CHA identifies what the health needs are, and the CHIP defines how the community will work to improve them.
