Death Certificates

Death Certificates are held in the county where the death occurred, regardless of where the person lived.

If you are a funeral home, please click here.

Ohio is an “open record” state, and vital records (births and deaths) are considered to be public records by the State of Ohio. This means that anyone who can submit the basic facts of a certificate may request a copy of it.

Death certificates are available three ways:

BY PHONE:

When you call, you will need to know the name (at the time of death) of the person you need a certificate for and their date of death.  The cost is $24.00 per certificate plus a $1.00 mail service fee payable by MasterCard, VISA, Discover, and Diners Club credit cards.

You may also use this form.  The cost is $24.00 per certificate plus a $1.00 mail service fee payable by check or money order made out to “MCHD”.

Funeral Homes

We are open to assist funeral homes Monday – Friday, 8:00 am – 4:30 pm.  In order to ensure that we have enough time to process your order, please arrive at least 15 minutes before closing.  Our death certificates are $24.00 per copy, and burial permits are $3.00.  Additionally, if you would like the certificate(s) returned to you via first-class, standard mail, you must include a $1.00 mail service fee.  When filing a death certificate, make sure they are complete and error free (no cross-outs, white-out, write overs, erasures).  Please remember that if the decedent served in the military, their family is entitled to a free, certified copy of the death certificate.  You may fill out and print this form to get the veteran’s copy.  If the death certificate you are filing has “pending” listed in the cause of death area, we will notify you when the supplemental certificates are ready to be picked up.

In order to file a certificate or an affidavit, you can come in to our office or mail it to us.  We process certificates and affidavits the day that we receive them.  If you would like the certificates mailed back to you, please include the $1.00 mail service fee.
