This form is used to report a POSITIVE UNPROCTORED at home COVID-19 test. An unproctored COVID-19 test means that you did not use the app or telehealth visit associated with the at home COVID-19 test kit used.

Who Should Get Tested?

If you begin experiencing symptoms of COVID-19, take an at-home (antigen) COVID-19 test. If you don’t have a test available or if you have other health/medical issues, contact your healthcare provider or the Medina County Health Center.

If you were exposed to someone with COVID-19, but do not have symptoms, keep your distance from others and watch for symptoms for 5 days after exposure. On day 6, take an at-home test.

Worried your at-home COVID-19 test might be expired? Here is more information about at-home COVID-19 diagnostic tests along with expiration dates and other details that may help you decide which test is right for you.
