Lodi United Methodist Church will be hosting summer childhood immunization clinics for children ages 0-18. Please register in advance.
Each child must be accompanied by a parent or legal guardian with a photo ID.
Please bring your child’s most current immunization record and any insurance card. The following forms of payment are accepted: most commercial insurances, Medicaid, cash, check, and credit cards. No child will be denied service due to inability to pay.
