You can obtain copies of birth or death records from the Medina County Health Department ONLY IF the birth or death  occurred within Medina County.

In the State of Ohio, birth and death records are filed in the county where the birth or death occurred.  You may also obtain Ohio birth or death records directly from the Ohio Department of Health.

Medina County birth and death records are available for all years from 1867 to the present.

 
For the years 1867 through 1908, please contact the Medina County Probate Court.

 
For the years 1909 to the present you can obtain a copy from the Medina County Health Dept. by mail, by phone, or by coming to our office.
YOU WILL NEED:
BY MAIL
  1. Name at birth or death
  2. Date of birth or death
  3. A signed and dated letter requesting the number of certificates you need
  4. A self-addressed, stamped envelope
  5. Payment:  A money order or personal/business check made payable to the Medina County Health Dept.  (Note:  Out-of-county personal/business checks will be held on deposit for ten business days before your request is processed.)

  3. Payment:  Cash, money order, personal/business check drawn on a Medina County bank, or credit card. VISA and MasterCard are accepted.   Make checks payable to Medina County Health Dept.   (Note:  Out-of-county personal/business checks will be held on deposit for ten business days before your request is processed.)
 
FEES

Effective October 16, 2009, the fee for a certified copy of a birth or death certificate will be $22 each.
Registrar, Theresa Schlauch, assists with a birth certificate request.
